Cuatro años de vigilancia epidemiológica de mpox en España, 2022-2025

Resumen

Introducción: En mayo de 2022 se identificaron los primeros casos de mpox en Europa y desde entonces se ha mantenido la transmisión en nuestro medio. El objetivo fue comparar las características clínico-epidemiológicas de los casos de mpox en España entre 2022-2025.

Método: Análisis descriptivo de los casos notificados a la Red Nacional de Vigilancia en Salud Pública entre 2022-2025. Se comparó la distribución de variables clínico-epidemiológicas según año de inicio de síntomas o de diagnóstico.

Resultados: Un total de 9.355 casos fueron notificados, de ellos 7.521 en 2022 (80,4%), 325 en 2023 (3,5%), 695 en 2024 (7,4%) y 814 en 2025 (8,7%). El 97,7% eran hombres y edad mediana de 37 años (RIC: 31-44). El porcentaje de nacidos en España varió entre 46,5% (2022) y 32,2% (2025). El 73,2% en 2022 presentaron síntomas generales frente al 62,1% de 2025, siendo la fiebre el síntoma más frecuente (55,4%). En global, el 65,3% presentó exantema anogenital y entre 4-6% requirieron hospitalización. Hubo cuatro fallecimientos, 3 en 2022 y uno en 2025. El principal modo de transmisión fue contacto estrecho en el marco de una relación sexual (66,6% en 2022 y 80,1% en 2025); entre 68%-78% eran hombres que tenían sexo con otros hombres. Entre 2023 y 2025, el 9,3% de los casos estaban vacunados frente a mpox. En total se han notificado 57 casos de clado I.

Conclusiones: Las características clínico-epidemiológicas se han mantenido globalmente estables desde 2022, con leves variaciones, en un contexto de aparición reciente de casos de clado I.
